If you choose not to submit your Host Family Application and Confidential Reference Form online, you can obtain the appropriate forms below.

The following documents are in Acrobat PDF format. If you cannot view these files, download the free Adobe Acrobat Reader. With it you'll be able to view and print the forms.

Confidential Reference Form (60K)As part of the application process, you will also be asked to submit two confidential references. These should be from close friends, neighbors, or employers, but not family members. Confidential Reference Forms should be returned to our San Francisco office by fax, email or mail. You can also submit the Confidential Reference form online.
